Early Years Support
Early Years Support aims to facilitate early intervention, information sharing and planning across services for babies /children under the age of 5 years where support may be needed regarding their learning/development and or physical/medical needs with a named lead professional.
Early Years Support recognises that some babies /children may need some additional support from education, health and care services at certain times in their life.  
The Early Years Support team aim to ensure children get the best start in life by offering advice and support at the beginning of their journey.
Within the East Riding of Yorkshire, Early Years Support is delivered through the Early Years Support coordinator and Early Years Support team.
The Early Years Support approach promotes:
•    Listening and responding to parents views
•    Parents and professionals working together to plan and review the support you and your baby/child need
•    Access to relevant information for families

Early Years Support is for families with a baby/child aged 0-5 years who have an additional need – diagnosed/undiagnosed.
Families must be living in the East Riding of Yorkshire and require support from one or more specialist services for example Paediatrician, speech and language therapy etc.

For children with a delay in their development or disability, families must be living in the East Riding of Yorkshire and require support from one or more specialist services e.g. paediatrician, portage.

General information
Next steps:
An initial date will be set between parents/carers and the person who made the request for Early Years Support to meet for an initial next steps discussion. This will be arranged at the family’s convenience and their preferred choice of venue.
The next steps discussion enables parents/carers to discuss any concerns they may have, ask any questions about the services recommended at the Early Years Support team meeting and plan the next steps together. The discussion also enables parents/carers to ask questions about the Early Years Support process.
A lead professional will be identified for the child at the next steps discussion. The lead professional will act as a single point of contact and will liaise with the family, Early Years Support and other services providing support. There will be the opportunity for parents/carers and all services to come together at an Early Years Support family meeting.
Further family meetings can be arranged as and when needed. These will include parents/carers, lead professional and all services working with and /or who may have been identified to work with the family to support the child.
The Early Years Support family meeting will provide an opportunity to look at family support, health and education needs which will be discussed and jointly agreed with the family as the next steps of support. This will then be formulated into an Early Years Support family plan and actions are agreed for people at the meeting to undertake within specific timescales.
