Description

The team provide assessment and intervention for children/young people with learning disabilities and associated significant emotional and behavioural needs.

The support may include direct work with the child/young person and/or work with parent/carers.

Who is the service for (ie threshold)

The team only works with children with a Learning Disability who are registered with an East Riding GP.

For children and young people with learning disabilities and associated significant emotional and behavioural needs, and this may include:

  • Behaviours that are challenging to others
  • Behaviours that cause harm to themselves
  • Anxiety and/or low mood that significantly impacts daily functioning.

They are a specialist service therefore will expect that universal and targeted services have been accessed.

Who can refer a child or young person?

Any professional involved with the young person, but not parents/young people directly.

How do you refer?

By referral form which can be obtained by emailing hnf-tr.childrenlearningdisabilityteam@nhs.net OR

No referral will be progressed without a consent form.

What happens after a referral is made?

The referrals are discussed every week at the referral meeting and a decision is made if further information is needed, a triage may then take place or it is placed on a waiting list.

A letter is sent to parents/carers/GP/school nurses and referrer to inform them of the outcome.

What happens if a child/young person is not accepted for treatment following a referral?

A letter is sent to the referrer and advice given regarding where an alternative service may be most appropriate to meet the person's needs.
