Looked After Children's Health Team

Description

Coordinate the Initial health assessment of each child and young person when they come into care and ensure health reviews are carried out, either six monthly for under 5 year olds and annually for 5 to 18 year olds. Health coordinators also work with any children and young people not in mainstream or special schools and those that reside in the two Local Authority children's homes and those who are preparing to leave care.

Registered service and regulatory body

Health coordinators and health visitors are regulated by the Nursing and Midwifery Council (NMC). Medical staff carrying out assessments are registered with the General Medical Council (GMC.)

Staff training and experience

The LAC coordinators are both registered nurses, one is a paediatric trained nurse and the other is Learning Difficulty trained. The designated nurse is also a registered nurse and health visitor.
