A range of services for children and young people including outpatient services, paediatricians, health teams for children in care and dental services.

Looked After Children’s Health Team
Coordinate the Initial health assessment of each child/young person when they come into care and ensure health reviews are carried out either 6 monthly for under 5 years and annually for 5-18 year olds. Health coordinators also work with any children/young people not in mainstream or special schools and those that reside in the 2 Local Authority children’s homes and those who are preparing to leave care.

Location and access
Health assessments can be carried out in the child/young person’s home or in school whichever is most appropriate, some assessments can be carried out in children’s centres. We carry out assessments all over the country as the East Riding LAC health team carry out statutory assessments of any children placed out of the county.

Access to this service
All children are initially referred into our service via the Local Authority, this is a targeted service only, however if a child/young person is “Looked After” then they can contact the team directly.

Children/young people are seen at venues suitable to them, this can be across the whole of the East Riding.
How we communicate with children, young people with SEND and their families
Communication is always with the carers alongside the child or young person if there are communication difficulties.
Registered service and regulatory body
The LAC health coordinators and health visitors are all regulated by the NMC.
Any medical staff carrying out assessments are registered with the GMC.

Training acquired to support children and young people with SEND and their families

The LAC coordinators are both registered nurses, one is a paediatric trained nurse and the other is Learning Difficulty trained. The Designated nurse is also a registered nurse and health visitor.
