General
Children's Learning Disability Community Team
Description
The Children’s Learning Disability Community Team is part of Humber NHS Foundation Trust.

They are a multidisciplinary team including nursing, psychology and psychiatry
The team provides assessment and intervention for children/young people with learning disabilities and associated significant emotional and behavioural needs.

The work will include positive behaviour support amongst other therapeutic approaches.

They also work closely with the Core CAMHS service.

Location and access
The service covers all of the East Riding of Yorkshire. Clinical interventions are delivered in childrens and young peoples homes as well as in a variety of settings eg Children’s Centres, GP practices, Schools.
Access to this service
The team currently accepts written referrals from professionals which may include paediatricians, GP, Social Care, School Nurse etc. Referrals either come directly to the team or via East Yorkshire CAMHS contact point. 

The team only work with children with a learning disability who are registered with an East Riding GP.

Who this service is for
For children/young people with learning disabilities and associated significant emotional and behavioural needs, this may include: 
• Behaviours that are challenging to others 
• Behaviours that cause harm to themselves 
• Anxiety and/or low mood that significantly impacts daily functioning 
 
The support may include direct work with the child/young person and/or work with parent carers. 
 
They are a specialist service therefore will expect that universal and targeted services will have been accessed
How we communicate with children, young people with SEND and their families
East Yorkshire Children's Learning Disability Community Team support families to communicate more effectively with children through visual aids and other resources.
Registered service and regulatory body
The service is registered with the Care Quality Commission (CQC). All the professionals within the team are registered with the appropriate body as follows:

  •     Nurses – Nursing and Midwifery Council
  •     Clinical Psychologists – Health Care Profession Council
  •     Psychiatry – General Medical Council

Training acquired to support children and young people with SEND and their families
Learning Disability nurses are registered Learning Disability Nurses.
Clinical Psychologists are trained to work with people across the age and ability range.
