The second link is a short survey asking about your experience of Short Breaks and the Freedom Activity Support Payment.

The first 10 responses for the short breaks survey will receive a Family swim voucher for any East Riding Leisure site, to be used by 30/11/17.
All entries received for both surveys will be entered into a prize draw to win a shopping voucher to be drawn on Monday 4/12/17.
